Disappearing extension icons

First, the CryptoData icon disappeared. Now, the uBlock Origin icon has disappeared. Both are nowhere to be found, and removing and reinstalling the extensions did not solve the problem.

How can I get these icons back?

In the future, how can I prevent extension icons from disappearing?

Have you tried right clicking on the toolbar and select "customize"?

Yes, I did, thank you for the suggestion. They are not in a toolbar, not on the row containing the address/URL bar (disappeared from there), not in the status bar, not in Customize. Nowhere to be found (at least, that I know of).

If you don't mind messing up the other icons (probably just be moved to Customize), you could try deleting localstore.rdf in your profile folder. It doesn't always work, and the next step may be a whole new profile. It's usually the restartless extensions that cause this trouble. No way to completely prevent if you use those extensions (and some are worse than others).
