also, print from icon toolbar gives error, print from file menu works ok, I feel that this toolbar is generally screwed up.

Did you try making a clean new profile? This includes not adding any addons (extensions), themes, bookmarks or customizations. Also, creating a new profile does "not" delete the present profile you are now using.

It's a known problem if you use old extensions based on the addon SDK (these can be identified by not requiring a restart after install, and not having any separate preferences/options dialog; there's just a preferences section for them in the addons manager.
Something to do with how they dynamically add their UI elements instead of being coded in static XUL.
I already use Cookie Masters and it caused no problems; Cookie Control panel on the other hand instantly added a button without prompting for a restart. So disable it and test your toolbar customizing again.I'd say there's no need for it if you already have Cookie Masters.

I've deleted all extensions and disabled adobe acrobat plugin. no change. I made a new profile and that fixes the problem in that profile. I'm not sure what to try next.

A clearer explanation: When I open "customise" the nov toolbar is normal. When I close customise bookmarks and history icons go away. These still work from the menu bar. Other things still work, address, search, forward, back, reload. So, pm still functions, just not from this toolbar.

I don't want to lose my many bookmarks so I would like to fix my default profile. Or I will continue to use in this slightly crippled state. Thanks.

Looks like you need to migrate to the new profile then. Have you looked at https://addons.palemoon.org/addon/febe/ ?

Thanks, FEBE looks promising.
New info: Turning off menu bar restores nav toobar to normal. If menu bar is on, nav toobar doesn't work. This is true in both default profile AND in new clean profile!
34.2.2 ---same.

I now have a better handle on the symptoms so I am going to start a new thread called:
menu bar interferes with nav bar
