The add-on - "Complete YouTube Saver".
I installed it yesterday, and the icon was there; the computer\browser probably crashed (I had a general problem - that I solved, sort-of). So probably - that's the reason the icon disappeared.
Note - I uninstalled the add-on, and reinstalled; and the add-on - remembers my download folder that I've set; so probably - it keeps the settings - and maybe that's what I need to delete and install again, to solve the problem.

Is the icon in your customize box?
If so, drag it out to where you want it.

No, it's not in the Customize-box or on hidden bars.

Fixed. It was quite weird: in the Customize-box - there was a button called CYS, which I realized was it; it was without an image. I dragged it to the bars, and still didn't see anything. Then I removed the mark from "show icon..." in the add-on, committed, and changed again to "show"; and now I see it.
BTW - it doesn't work; just stays grayed-out.
