Using Pale Moon 27.4.2 on Windows 7 Home Premium.
Began seeing a double image in the address/URL bar several versions ago. Has anyone else experienced this?

This is usually due to an incompatibility with the theme you are using.

Install Lootyhoof's Theme Compatibility Provider, and see if that solves problem.

Just out of curiosity, which theme are you using?

Hmm, that looks like "Firefox 3 theme for Firefox 4+ Reloaded." That has been forked for Pale Moon (as Moonfox3), along with a bunch of other themes, located here:

As an aside, this problem should go away with 27.5 since the root cause of this (the status overlay layer for status in the address bar) will no longer be used.
It'll be a positive side effect that it will make Firefox themes a little bit more compatible out of the box because of it.

This theme is definitely "Firefox 3 theme for Firefox 4+ Reloaded". I forked this a long while ago as Moonfox3, as mentioned above.

Many other things apart from this are fixed in it, as well as improving the overall Firefox 3 feel, so I'd suggest you take a look. :)

I was using "Firefox 3 for Firefox 4+ Reloaded!" and it was incompatible with 27.4.2
